America doesn't introduce himself as Alfred or really tell anyone unless they ask, but he's not super duper secretive about it. In relation to Canada, in my mind, he calls him Matt a lot because, again, he grew up with basically him, a bunch of humans, and England. England himself is a more messy case with using human names because I don't think America's called him Arthur to his face, and if he did it wasn't in the casual "oh I hung out with Arthur today" sort of way.
In a way, it's like having different but equal standards of intimacy between America and his family.
If America ever finds out Russia's name he's going to call him it to his face. And he's going to Anglicanize it. But, to America, it doesn't quite have the same consequences if Russia calls him Alfred. It will just be another FUCKING RUSSIA point.
