Medical Records Vault
To locate and update In-Character (IC) medical records easier, I've decided to make this organizational post.
How it will work:
1. Your character fills out the medical form ICLY.
2. I comment with the information given under their FIRST NAME.
3. If information needs updating or noted, a reply can be submitted as a comment to the character's information.
YOU ARE NOT REQUIRED TO COMMENT/REPLY TO ANYTHING.
I will comment with your character's information after they have submitted it ICLY.
Due to not all characters having last names (Like Cube! lmao) Everything will be organized by character's FIRST NAME. In the event a NICK NAME is used, information will be filed under that name unless otherwise noted.
IC notes can be added by simply replying to a character's medical form.

2. What species are you? Red-Eared Slider Turtle.
3. Name of place of origin? Earth.
4. What is your gender? Male.
5. What is your age? 47.
6. Any allergies or special needs? Can't stand anchovies.
7. Medical history? I've lost an arm and been treated for a bunch of different injuries. I've had my shots, though.
8. Have you gotten sick since your arrival here? No. Died, though.
9. Blood Type? I can't give or receive blood so it doesn't matter.
10. Have you been in Discedo before?
A. If so, how long was your stay? I've gone to Discedo a few times but never for more than a few hours, minus the tunnel work only recently which spanned a few days.
11. How many times have you died here? Once. Morbid question.
12. Do you currently have your chip? Yes.
13. If not, then how long ago was it removed and by who?
A. How did everything go? Did your powers effect the city?
14. What abilities do you possess? Ninjutsu mastery. Nunchaku mastery. Heightened reflexes, stealth and speed. I also have a lot of stamina and a very high pain tolerance. It takes a lot to keep me down.
